Run-time strategy for data errors

Although efforts are made to create and maintain an 'error-free' database, numerous minor logical inconsistencies may persist during correspondence with trialists. In addition to the Diktats concerning missing dates etc, the following precepts are applied globally at run-time:

In addition to cases with 'distant/unknown-site recurrence' flags set, an unknown-site recurrence is taken to be present if a date of distant/unknown recurrence is present.
In addition to cases with 'locoregional recurrence' flags set, a locoregional recurrence is taken to be present if a date of distant/unknown recurrence is present.
In addition to cases with 'contralateral breast cancer' flags set, contralateral breast cancer is taken to be present if a date of contralateral breast cancer exists, except that if reported on (or before) the randomisation date (if non-null) it is disregarded throughout (with the exception of the 'CONT' endpoint, for which such cases are censored at the randomisation date).
In addition to cases with 'second malignancy (ICD)' reported, a second malignancy at unspecified site is taken to be present if a date of second malignancy is present.

CONT: CONTRALATERAL BREAST CANCER AS FIRST EVENT

An event is taken to have occurred if:

contralateral breast cancer is reported; and

the date of contralateral breast cancer does not exist and a recurrence is not reported; or
the date of contralateral breast cancer exists; and

a date of first recurrence does not exist or the date of first recurrence is on or later than the date of contralateral breast cancer; and
a date of first distant recurrence does not exist or the date of first distant recurrence is on or later than the date of contralateral breast cancer.

The event date is taken as the date of contralateral breast cancer.

If an event is not taken to have occurred, the case is censored at the randomisation date if the date of contralateral breast cancer is on or before the randomisation date, or else at THE EARLIEST OF the date died or last traced, the date of first recurrence (if non-null) and the date of first distant recurrence (if non-null).

DD: MORTALITY

An event is taken to have occurred if:

death has occurred.

The event date is taken as the date died.

If an event is not taken to have occurred, the case is censored at the date last traced.

DDPOSTREC: MORTALITY OMITTING DEATHS FROM NON-BREAST-CANCER CAUSES WITHOUT RECURRENCE

This endpoint is formed by subtraction of the annual events, (O - E) and variance of NONBC from DD, q.v.

DDUKRL: MORTALITY FROM UNKNOWN CAUSES WITHOUT RECORDED RECURRENCE

An event is taken to have occurred if:

death has occurred; and
the cause of death is 'unknown' or is not stated; and
a recurrence is not reported; and
contralateral breast cancer is not reported.

The event date is taken as the date died.

If an event is not taken to have occurred, the case is censored at the earliest of the date last traced, the date of first recurrence (if non-null), the date of first distant recurrence (if non-null) and the date of contralateral breast cancer (if non-null).

DIST: DISTANT RECURRENCE

An event is taken to have occurred if:

neither a recurrence nor contralateral breast cancer are reported and death has occurred from 'breast cancer'; or
a distant recurrence is reported and contralateral breast cancer is not reported; or
a date of first distant recurrence exists and the date of contralateral breast cancer exists and is after the date of first distant recurrence.

The event date is taken as the date of first distant recurrence (if any) or else the date died (if dead without recurrence) OR ELSE THE DATE DIED OR LAST TRACED (IF THE DATES OF BOTH FIRST RECURRENCE AND FIRST DISTANT RECURRENCE ARE UNKNOWN).

If an event is not taken to have occurred, the case is censored at the earliest of the date died or last traced, the date of contralateral breast cancer (if non-null) and, if death has occurred from 'unknown' or unstated cause, the date of first recurrence (if non-null).

DDENDO: ENDOMETRIAL CANCER MORTALITY

An event is taken to have occurred if:

death has occurred; and
the cause of death is 'other second neoplasm'; and
the death cause text begins '(d. endometrial' or ICD-7 codes 173, 174 or 233, ICD-8 codes 182 or 234 or ICD-9 code 182 are present; and
a recurrence is not reported; and
contralateral breast cancer is not reported.

The event date is taken as the date died.

HEART: CARDIAC MORTALITY

An event is taken to have occurred if:

death has occurred; and
the cause of death is 'heart'; and
a recurrence is not reported; and
contralateral breast cancer is not reported.

The event date is taken as the date died.

LEUK: MORTALITY FROM LYMPHATIC AND HAEMATOPOIETIC CAUSES

An event is taken to have occurred if: death has occurred; and the cause of death is 'lymphatic and haematopoietic'; and a recurrence is not reported; and contralateral breast cancer is not reported. The event date is taken as the date died. If an event is not taken to have occurred, the case is censored at the earliest of the date last traced, the date of first recurrence (if non-null), the date of first distant recurrence (if non-null) and the date of contralateral breast cancer (if non-null).

NONBC: MORTALITY FROM NON-BREAST-CANCER CAUSES WITHOUT RECURRENCE

An event is taken to have occurred if:

death has occurred; and
the cause of death is 'acute treatment toxicity', 'infective', 'lymphatic and haematopoietic', 'other second neoplasm', 'heart', 'thrombotic or embolic', 'other vascular', 'extraneous', 'not 1-8 and not breast cancer' or 'not breast cancer'; and
a recurrence is not reported; and
contralateral breast cancer is not reported.

The event date is taken as the date died.

NONBCUK: MORTALITY FROM UNKNOWN NON-BREAST-CANCER CAUSES

An event is taken to have occurred if: death has occurred; and the cause of death is 'not 1-8 and not breast cancer' or 'not breast cancer'; and a recurrence is not reported; and contralateral breast cancer is not reported. The event date is taken as the date died. If an event is not taken to have occurred, the case is censored at the earliest of the date last traced, the date of first recurrence (if non-null), the date of first distant recurrence (if non-null) and the date of contralateral breast cancer (if non-null).

NONBCVASC: MORTALITY FROM NON-BREAST-CANCER, NON-VASCULAR CAUSES

An event is taken to have occurred if:

death has occurred; and
the cause of death is 'acute treatment toxicity', 'infective', 'lymphatic and haematopoietic', 'other second neoplasm', 'extraneous' or 'not 1-8 and not breast cancer'; and

A DISTANT RECURRENCE IS REPORTED AND CONTRALATERAL BREAST CANCER IS NOT REPORTED AND THE DATES OF FIRST RECURRENCE AND FIRST DISTANT RECURRENCE ARE BOTH UNKNOWN; OR
dates of first recurrence and first distant recurrence exist and the date of first distant recurrence is on or up to 7 days after the date of first recurrence; and

contralateral breast cancer is not reported; or

neither a recurrence nor contralateral breast cancer are reported and death has occurred from 'breast cancer'; or

An event is taken to have occurred if:

a recurrence is reported and the trial/stratum does not lack recurrence site information; and

a first distant recurrence is not reported; or
dates of first recurrence and first distant recurrence exist and the date of first distant recurrence is more than 7 days after the date of first recurrence;

This endpoint is formed by subtraction of the annual events, (O - E) and variance of RLLI from RLF, q.v.

RLU: RECURRENCE AT UNKNOWN SITE AS FIRST EVENT

An event is taken to have occurred if:

neither a recurrence nor contralateral breast cancer are reported and the trial/stratum lacks recurrence site information and death has occurred from 'breast cancer'; or
